What you need to register
- Your personal contact information including occupation
- Emergency contact information
- Insurance information including group name and number, policy number, address, and telephone number
- Physician name
- Date of procedure or appointment
Additional information needed from surgery patients:
- Name(s), address(es) and phone number(s) of your physicians
- List of current medications with dosage and frequency
- List of all surgical procedures you've had
- Surgeon name
